Clinical Tools' Remote Live Standardized Patient (SP) Framework
Need: Physicians and physicians-in-training need a standardized patient experience for learning and evaluation that is convenient and easy-to-use, and retains many the unique ability of a face-to-face standardized patient encounter to practice and assess skills.
Vision: Remote Live Standardized Patients will enhance current skills training education and provide the target audience with an effective and engaging experiential learning opportunity.
Mission: To create online skills training for the target audience that is innovative, interactive, cost-effective, and models real-world experiences via remote live standardized patients.
Goals: Incorporate remote (off-site) Standardized Patients within on-line educational modules as training elements to enhance skills training.
Learning Objectives: Learning objectives will be different based on learner target audience. In general, we will be able to document change in learner performance in the following:
- Identifying the patient problem
- Screening the patient
- Diagnosing the patient
- Developing a treatment plan
- Explaining the treatment plan to the patient
Assessments: Each health professional or student will be rated post-interview based upon pre-selected scoring criteria. Two independent raters will be used for each encounter.

CTI SP Roles
To create and carry out a remote Standardized Patient encounter, we typically have the following roles:
- Writer: The writer is responsible for creating a script for the standardized patient. Ideally, the script will follow a checklist which will be used to evaluate the learner. See CTI SP Script page for guidelines for writing a good script.
- Standardized Patient (Actor): Currently, the SP is a CTI employee. Eventually, we hope to have users become SPs. For tips on being the actor, see CTI SP Instructions for Patient (Actor) page.
- Moderator: The moderator is someone who monitors the chat. The moderator will invite physicians to chat and monitor time elapsed. The moderator will also rate the physician.
- Evaluator: The evaluator is currently the actor. The evaluator also rates the patient.

CTI Learner Target Audiences
Need: We have conducted several needs analyses (see SP Needs Analysis) to determine how to best educate our target audiences.
Target Audience: These are the target audiences we are currently working with.
- Medical Students: We are creating a page (CTI SP Instructions for the Medical Student Learner) that will help the learner understand what is expected of them.
- Resident/Fellow Learner: We will advise residents/fellows about how they can benefit from the SP encounter. See CTI SP Instructions for the Health Professional
- Practicing Doc Learner: We will advise physicians about how they can benefit from the SP encounter. See CTI SP Instructions for the Health Professional

CTI Evaluator Target Audiences
These will be the people who will need to understand how the remote SP works:
- Reviewer (Content/Primary Care): - We will contact project/SP expert consultants to review our cases. See CTI SP Consultants for a list of consultants who are experts in Standardized Patients.
- Course Director (MS): We need a way to explain the remote SP concept and benefits (such as evaluations, patient notes) to MS course directors.
- Clinic Director (Practicing): We need a way to explain the remote SP concept and benefits to clinical directors.
- 3rd Party (VA, ASAM): Potential commercialization here
- Grant Reader: Whether a contract or a grant, we need to work on the CTI SP Background Research page to include in future grants/contract applications.
- Grant Report Reader: When writing contract monthly reports or grant end of year reports, we need to find a way to explain our current methodology, backed up by needs analyses and usability.
