Patient Introduction

Name: Cynthia Madison Stewart

Presenting Situation

Chief Complaint: difficulty sleeping/insomnia, fatigue, some headaches, anxiety, loss of appetite--has been going on for about 3 months now.

Narrative: Cynthia Madison Stewart is a 40 year old Caucasian woman

Family History: Mother, father, sister, and brother are all alive, no major health issues.

Social History: Stable home life, single, past relationships failed due to job stress.


Physical Exam

Description: Adult Female, 40 years old

Height: 5'5"

Current Weight: 145 (Max/min over the past year: 150/140, no recent excessive gain/loss, but loss of appetite at times)

No difficulty swallowing, normal bowel behavior, not sensitive to any foods, does not smoke or use drugs, is not currently on any medications except for OTC medicines to deal with her insomnia and headaches, and caffeine intake is mainly through coffee and soft drinks. Has not had a past history of any physical disorders, but extended family has a history of cancer and heart problems. No hospitalizations recently, the last time was when she broke her ankle at the age of 27.
