MS Alcohol Standardized Patient Framework

SP Information
Project: MS EtOH (Summative Evaluation)
Need: We need a Standardized Patient case that will allow us to evaluate if a medical student appropriately interviews, reviews patient history, screens, diagnoses, and develops a treatment plan for a patient with an Alcohol Use Disorder (AUD), based on the best practices presented in our MS Alcohol curriculum.
Vision: The Standardized Patient case will allow students to demonstrate their newly acquired knowledge and implement new clinical skills in a time-efficient manner that can be measured using typical assessment instruments.
Mission: The instructor/evaluator will be able to measure change in students' clinical skills, specifically skills relating to patient interview/diagnosis/treatment planning skills by participating in a Standardizdd Patient clinical encounter. Prior to taking the Medical Student alcohol education modules, a student will likely not be able to appropriately interview, diagnose, and develop a treatment plan for a Standardized Patient with an Alcohol Use Disorder. After taking the educational modules, the student will demonstrate these skills via a successful Standardized Patient interview.
Target Audience: Medical students who will be recruited in our summative evaluation. Medical educators who we might recruit to have their students take our modules (summative evaluation or later).
Goal: Medical students will be able to demonstrate appropriate screening, diagnoses, and development of treatment plans for a Standardized Patient with an alcohol use disorder. As educators, we will be able to document the level of student performance before/after the Standardized Patient interview with reference to key learning objectives from the curriculum as a whole.
Objectives
We will be able to document change in student performance of:
- Identifying common warning signs and symptoms of alcohol use disorders (Learning Objective #3, Scope of the Problem course)
- Screening the patient with an appropriate AUD screening tool (Learning Objective #2, Screening, Assessment and Diagnosis course)
- Diagnosing a patient with abuse or dependence (Learning Objective #3, Screening, Assessment and Diagnosis course)
- Diagnosing a medical complication that can arise from increased alcohol use (Learning Objective #1, Medical Psychiatric and Polysubstance Comorbidities course)
- Developing a treatment plan for the patient (Learning Objective #1, Brief Intervention and Treatment Referral course)
- Explaining treatment decisions and treatment confidentiality with patients as part of informed consent procedures (Learning Objective #3, Role of the Physician course)
Implementation and Evaluation
Implementation: We will conduct remote SP interviews (using Google Chat) at the beginning and end-points of the participants' experience in the summative evaluation.
Evaluation: We will create a rater form to evaluate clinical skills and basic communication (professionalism).
